Personal information protection
To be a healthcare provider deeply trusted by patients and society, we constantly endeavor to provide high-quality medical care.As part of such efforts, we place great importance on appropriate protection and management of our patients’ personal information.Toward these goals, we have formulated the following personal information protection policies. We will work to ensure thorough implementation of these policies.
- 1. Collection of personal information
When we collect our patients’ personal information, we do so within the scope necessary for examining, nursing, and treating patients. (See “The hospital’s purposes of use of personal information.”) If we wish to use a patient’s personal information for purposes other than these, we will inform the patient of the purposes of use in advance and obtain their consent before using such information.
- 2. Use and provision of personal information
Except in the following cases, we will not use our patients’ personal information for purposes other than the original purpose of use.-Cases where we have obtained prior consent from patients.
-Cases where personal information is processed in a form in which individual identification is not possible.＊1
-Cases where provision of personal information is required by law
Except in cases specified by law, we will not provide to any third party＊2 any personal information of patients without their prior consent.
＊1 “Form in which individual identification is not possible” not only means a form in which anonymization of relevant personal information has been achieved by erasing an individual person’s name from said personal information, but also a form in which no personal identity can be established by any means.
＊2 “Third party” means organizations or individuals other than the hospital and patients themselves that do not meet the original purposes of use of the personal information in question or to which or to whom a patient has not given approval for the use of such personal information.
- 3. Appropriate management of personal information
We make sure to keep patients’ personal information in an accurate and updated condition and prevent the leakage, loss, and falsification of such personal information or unauthorized access to said information.
- 4. Verification, correction, etc. of personal information
If a patient asks us to disclose his or her personal information to him or her, we will promptly verify the content of said information and take action in compliance with our “Guidelines on the Provision, etc. of Patient Information.” Also, if we are asked to correct the content of any personal information on account of the inaccuracy of such information, we will conduct relevant investigation and take appropriate action.

- 6. Improvement of mechanisms for compliance and personal information protection
We comply with Japanese laws, Ministry of Health, Labour and Welfare guidelines, rules and practices followed in the medical and healthcare fields, and other relevant rules and regulations concerning personal information protection. Also, we review each item in the above rules and regulations from time to time, thereby ensuring continuous improvement of our personal information protection mechanisms.
The hospital's purposes of use of personal information
We exercise sufficient caution in handling patients’ personal information. We use patients’ personal information for the purposes below.
- 1. Provision for medical purposes
– Provision of medical services at the hospital
– Collaboration with other hospitals, clinics, midwife clinics, pharmacies, home nursing stations, and long-term care service providers
– Response to inquiries from other medical institutions
– When requesting opinions or advice from doctors or other medical professionals from outside the hospital on medical care for patients
– Outsourcing of sample testing and other services
– Explaining patients’ conditions to their families
– Other uses concerning the provision of medical care to patients
- 2. Claim processing procedures
– Office procedures we carry out with regard to medical, nursing-care, and workers’ accident compensation insurance and publicly funded healthcare services, as well as the outsourcing of such office procedures
– Provision of receipts to claim processing and payment organizations
– Response to inquiries from claim processing and payment organizations or insurers
– Provision of receipts to administrative agencies and responses to their inquiries concerning publicly funded healthcare services
– Other uses relating to claim processing concerning medical, nursing-care, and workers’ accident compensation insurance and publicly funded healthcare services
- 3. The hospital’s administration procedures
– Accounting and bookkeeping
– Reporting of malpractice, etc.
– Improvement of medical services to the patient concerned
– Admission, discharge, and other hospital bed management procedures
– Uses relating to the hospital’s other administrative procedures
- 4. For processing the medical records / check-up results of the members of a particular company or institution by which the medical check-ups were done outside the hospital upon permission
- 5. Consultation with, or submission of notification to, specialized medical organizations, insurance companies, etc. with regard to medical professional liability insurance
- 6. Provision of basic materials for the maintenance and improvement of medical and nursing-care services and procedures
- 7. Cooperation for medical training conducted in the hospital
- 8. Case studies conducted within the hospital for improving the quality of medical care
- 9. Provision of information to the nosocomial cancer registry, which needs to be administered by a Cancer Treatment Affiliate Activity Base Hospital and provision of information to related external organizations
- 10. Provision of information to external audit organizations
If you do not consent to providing your information to other medical or other institutions for any of the above-listed purposes, please provide notice to that effect to the section in charge. Without such notice, you will be deemed to have given consent to the uses of information listed above. You may withdraw or change such notice of yours at any time.
